The form below is used to collect the data that we need to do the administrative function for a Charitable Remainder Trust.

It may be a good idea to give us a call (888) 343-3132 before a trust is drafted and funded to make sure the proper steps are followed.

A Trust Administration Agreement should be signed before the following form is begun. The appropriate fee, as stated in the agreement, should be sent with the contract. However, call us for help putting the transaction together.

The form can be filled out in steps. For example, the donor's name, address, etc. on the first page can be sent to us when a trust is just about to be completed.
